d9 in smoke

wondering what will make a d9 blow the outputs, been hooked up awhile and went to turn headunit on the other day and i hear a pop from the amp, turn the headunit off and back on it started smoking turned it off and opened it up and three of the fets on the same side of the slave switch was blown,

i have on the 12volt side 2 kinetik 2400 and on the 16volt side 6 xs power d1600 voltage on the 16volt side was resting at 17.20 and on the 12volt side i dont know the vechile wasnt running, just listening to the music, only thing on the 12volt was the remote, so couldve been low voltage then on the remote
